Fascist Italian Mother’s Medal, the “Medaglia d’onore per le madri di famiglie numerose”. Just like the German Mother’s Cross, it was established by Benito Mussolini’s regime as part of his “Battle for Births” campaign, which aimed to increase the Italian population. Seven bows indicates 7 children. Much less common that it’s German equivalent.
